Welcome to the 2019-2020 Dance Season!

Upon registration of your child(ren), you are occupying and reserving a space(s) in a class/classes. 

In doing so, you are committed to, and agree to pay monthly tuition for said class/classes, including a registration fee and any costume fees if applicable, throughout the entire 2019-2020 season.

 If the situation arises that you can no longer keep this commitment, YOU MUST FILL OUT A DROP FORM 30 DAYS PRIOR TO EXITING THE CONTRACT. 

Please Note: That policy will be in the “Competing Group Drop Policy” section of this contract. 

The first day of classes begins August 5th, 2019 with the last day of classes being May 16th, 2020.

This season consists of 10 months. The season is not calculated by weeks, but rather by how many classes are taken over a 10 month period and how many hours a student(s) attends in a week. Tuition is counted on an hourly rate and also an annual accumulation.

Financial Commitments


  • Registration Fee(s  & Tuition:

Each student will be charged a non-refundable annual registration fee due upon registration. Registration fees are not prorated. The registration fee proceeds as follows: $15/per one student and $10/per each student added, within the same immediate family. Initial:  ____________ 


Tuition is to be paid on a month to month basis. Tuition is due the month prior to enrolled month, meaning tuition is ran on the 25th of each month, with the last day to pay tuition being the 1st of the enrolled month. Late fees apply on the 2nd of the month in the amount of $20.00. THERE IS NO EXCEPTION.

  •  Recreation Class Fees:

The Recreational Program extra fees consist of the following: Registration Fee(s)  (upon initial registration), Costume Fee(s) and a Spring Recital Fee. If a costume is lost and a replacement is needed, Element Dance Center is not responsible for that cost.  A breakdown has been given below. 


Costume Fees

Ballet, Jazz or Tap Costume $80.00 $40.00 due Sept. 13th $40.00 due Nov. 15th

Hip Hop Costume $60.00 $40.00 due Sept. 13th $20.00 due Nov. 15th

Break Dance $20.00 $20.00 due Sept. 13th

*Recital Fee only includes 2 parent tickets, and $5.00 spectator tickets will be for sale at the entrance. Initial:  ____________ 


  • Dropping Classes / Terminating Contract for Recreation Classes: 

Element Dance Center requires a 30 day notice when dropping a class in our recreational program. If a 30 day notice is not given, tuition for the upcoming month is still due. This must be a written notice. If it is communicated via email, text or voicemail, we DO NOT consider that a drop. The Parent/Guardian MUST stop into our office during business hours and fill our a drop form to be considered fully dropped from the class(es) in which they are enrolled. Initial:  ____________ 


  • Competing Group Drop Policy:

If the client attended the audition, an online commitment was made through Studio Director. This means a spot was taken in our program and if a dancer terminates that position, this can cause several budgeting issues. The client will be liable to pay the tuition for the month's remaining in the season and any other fees that may have been accrued. Initial:  ____________ 


Winter and Spring Recitals


  • Winter Progression Performance: 

Element Dance Center will be hosting an in-class Winter Progression Performance for our Recreation Program. This will be held in our large room the week of January 20th - January 26th 2019. This will be an intimate opportunity for parents to see the progress the dancers have made. There is no extra cost for this and no entrance fee. Initial:  ____________ 


  • Spring Recital:

Element Dance Center will be hosting our Annual Spring Recital on May 16th, 2019. The Recital Fee(s) were discussed in the “Financial Commitment” section of this contract. Initial:  ____________ 


Snacks, Water, & Leaving the Premises


  • Element Dance Center Snacks: 

Element Dance Center has snacks for sale. Snacks can be added to the clients Studio Director Account and the balance will be added to the tuition ran on the 25th of each month. If the student is paying in cash, this procedure will not apply. This is only when a student does not have cash. Initial:  ____________ 


  • Water & Drinks:

In an effort to become more environmentally mindful, we will no longer be supplying drinking cups, nor will we be carrying plastic water bottles. Parents and Dancers will be responsible to supply their own drinking bottle. Initial:  ____________ 


  • Permission to Leave Premises

As well, students over the age of ten are allowed to leave the studio to visit the surrounding businesses in groups of three or more without adult supervision, but ONLY if their parent or guardian consents. ELEMENT DANCE CENTER IS NOT RESPONSIBLE OR LIABLE FOR ANY OCCURRENCES THAT MAY HAPPEN WHEN STUDENTS LEAVE THE PREMISES. Dress Code and Class Etiquette


  • Class Attendance and Tardy Policy:

Element Dance Center values punctuality. Being on time is our expectation. If there is an emergency, the best form of communication is to text Element Dance Center at (801) 834-2422. Initial:  ____________ 


  • Behavioral Issues or Special Needs:

If your child has a behavioral issue or special need, please advise the front desk. Initial:  ____________ 


  • Injury:

If your child has an injury or illness that a physician has suggested he or she be limited from physical activity, you must provide a doctor’s note to hold a space in a class without billing AND your child must be cleared by the same physician in writing to return to dance. If your child has a cast, crutches, braces etc., your child will not be allowed to participate in dance without written clearance from a physician. No refunds or credits will be given for injuries after the child is authorized to return to class.

  • Consecutive Absences:

If your child is in a full class and your child has not attended the class for 3 consecutive weeks without a parent request and payment to maintain his or her space, your child may be dropped from a class to allow others to participate without a refund or credit for tuition paid or owed. The client will still be charged for the month(s) in which the child is enrolled and not officially dropped by the parent. Initial:  ____________ 


  • Class Make Up Policy:

Any missed classes may be made up by attending a class of the same level or lower and in the same age range only within the same calendar month. If the dancer misses classes in the last week of a calendar month, the dancer may make up the class in the following calendar month. No credits or refunds are given for missed classes. Element Dance Center is not responsible for tracking missed classes. Students may not make up more than twice in the exact same class. To make up a class, you must schedule it through the secretary at the front desk. There are no makeups for classes missed because of holidays, for unlimited dancers, or for classes missed in a past calendar month. Makeups cannot be done by siblings or friends.   • Dress Code:

Ballet classes for Company & Team members must be in full ballet attire. This includes; a black leo, ballet pink tights, and ballet pink canvas shoes. Hair must be in a high ballet bun for females. Male dress code is as follows: black tight-compressed top, black male ballet tights, (NO BASKETBALL SHORTS. If this is uncomfortable for your dancer, I would recommend a dance belt) and black canvas ballet shoes.

All other classes are as follows: Ballet/Jazz = ballet leotard, tights, and ballet shoes, Tap = dance clothes with black tap shoes (unless the teacher has decided on another color).  Hip Hop/Break Dance = clothes that allow movement and sneakers. Acro/Tumbling = tight dance clothes and bare feet. Cheer = dance shorts, tight top, and sneakers. Lyrical/Jazz Classes = dance clothes and pirouettes. Initial:  _________

Studio Communication


  • Email and Text Communication:

MAKE SURE YOU ARE OPTED IN FOR EMAILS AND TEXT MESSAGES. All information regarding the studio, (including cancelled classes, snow days, extra performances/practices, etc.) will be communicated via these two methods. By signing this contract, you agree to these communication terms, will provide accurate contact information, and will frequently check for any information being sent to you. Initial:  ____________ 


  • Phone, Email and Client Communication to Element Dance Center:

Our hours of operation are as follows: Monday’s & Tuesday’s from 4pm-8:30pm, Wednesday’s from 4:30pm-9pm, and Thursday’s from 5:00pm-8pm. If you need to reach us on the telephone, please note that we are not typically at the studio during the day so your phone call will most likely not be answered. Please leave a voicemail. I, _________________________________, the parent of __________________________________, (Full Names, Please Print), HEREBY ASSUME ALL OF THE RISKS OF PARTICIPATING IN ANY/ALL ACTIVITIES ASSOCIATED WITH ATTENDING ANY AND ALL ELEMENT DANCE CENTER EVENTS, including by way of example and not limitation, any risks that may arise from negligence or carelessness on the part of ELEMENT DANCE CENTER, and/or their directors, officers, employees, volunteers, representatives, agents, activity holders, sponsors, and organizers, and from dangerous or defective equipment or property owned, maintained, or controlled by ELEMENT DANCE CENTER, and/or their directors, officers, employees, volunteers, representatives, agents, activity holders, sponsors, and organizers, or because of their possible liability without fault. 


I certify that my child and/or I are physically fit, have sufficiently prepared or trained for participation in this activity, and have not been advised to not participate by a qualified medical professional. I certify that there are no health-related reasons or problems which preclude my participation in this activity. 


I acknowledge that this Accident Waiver and Release of Liability Form will be used by ELEMENT DANCE CENTER, and/or their directors, officers, employees, volunteers, representatives, agents, activity holders, sponsors, and organizers of the activity in which I may participate, and that it will govern my actions and responsibilities at said activity.


In consideration of my application and permitting me to participate in this activity, I hereby take action for myself, my executors, administrators, heirs, next of kin, successors, and assigns as follows: 

(A) I WAIVE, RELEASE, AND DISCHARGE from any and all liability, including but not limited to, liability arising from the negligence or fault of the entities or persons released, for my death, disability, personal injury, property damage, property theft, or actions of any kind which may hereafter occur to me, including my traveling to and from this activity, from ELEMENT DANCE CENTER, and/or their directors, officers, employees, volunteers, representatives, agents, activity holders, sponsors, and organizers.

(B) I INDEMNIFY, HOLD HARMLESS, AND PROMISE NOT TO SUE the entities or persons mentioned in the above paragraph from any and all liabilities or claims made as a result of participation in this activity, whether caused by the negligence of release or otherwise. 


I acknowledge that ELEMENT DANCE CENTER, and/or their directors, officers, employees, volunteers, representatives, agents, activity holders, sponsors, and organizers are NOT responsible for errors, omissions, acts, or failures to act of any party or entity conducting a specific activity on their behalf. 


I hereby consent to receive medical treatment which may be deemed advisable in the event of injury, accident, and/or illness during this activity. 


I understand while participating in this activity, I may be photographed. I agree to allow my photo, video, or film likeness to be used for any legitimate purpose by ELEMENT DANCE CENTER, and/or their directors, officers, employees, volunteers, representatives, agents, activity holders, sponsors, and organizers.


The Accident Waiver and Release of Liability Form shall be construed broadly to provide a release and waiver to the maximum extent permissible under applicable law.
